Referral Information

What to include with a referral

Areas of practice

Trauma, hip and knee conditions, arthritis, sports injuries, joint replacement and revision surgery.

Clinical history

Relevant history, examination findings and prior treatment attempted.

Previous treatment

Details of any prior surgery, injections or physiotherapy relevant to the presenting problem.

Imaging

Any relevant imaging or reports, attached to the referral where available.

Urgency

Please indicate the clinical urgency of the referral so it can be triaged appropriately.

Contact information

Your name, practice and direct contact details, so a clinical summary can be returned to you.

Urgent referrals

For time-sensitive referrals, please call the practice directly to discuss the case. For emergencies, direct the patient to the nearest emergency department rather than relying on a written referral alone.

Imaging Requirements

Including imaging with a referral

Where imaging has already been performed, please include reports and, where possible, image access details with the referral. Specific imaging protocols vary by presentation and will be advised on request or at consultation.
